1. notify the Insurance company сповістити about the fact within 24 hours (by telephone, fax, telegram) by telephone numbers specified in the health insurance policy;
2 apply for medical assistance (service) to the medical establishment from the list of medical establishments determined by the insurance policy. Medical assistance (service) is provided to the Insured on the basis of the presented insurance policy (or the document confirming the fact that the insurance contract was concluded) and the document identifying the Insured;
3. receive the medical assistance (service) prescribed by the medical establishment;
4. make sure the medical establishment has drawn all the necessary medical documents;
5. after the medical assistance (service) has been obtained receive in the medical establishment documents certified by the Head doctor and the seal of the medical establishment and submit them to the Insurer.
